چکیده

In an interesting and original article in a recent issue of Critical Care, Oshiro and colleagues describe the profile of acute coagulopathy of trauma-shock (ACOTS) using viscoelastic techniques [1]. The authors, in light of their results, demonstrate the hypothesis that ACOTS is a type of disseminated intravascular coagulation (DIC) with a fibrinolytic profile. However, we would like to highlight some aspects we consider to be of interest. It has caught our attention that, fulfilling national laws, patients were not subjected to fluid resuscitation during transport to the hospital. We understand that aggressive fluid resuscitation is deleterious to the patient and induces the possibility of dilutional coagulopathy [2–4]. However, despite the absence of strong evidence in this respect, current guidelines recommend its administra-

Atraumatic rupture of the spleen is an uncommon but important clinical entity. Unfortunately, the diagnosis is often missed due to the absence of any history of trauma. Atraumatic rupture of the spleen has been described in the medical literature as a clinical oddity with grave consequences, if it is unrecognised and untreated. Clinical features of splenic rupture include pain, tenderness a...

O18: In Hospital Management of Child with Road Trauma

Trauma is the most common cause of mortality and morbidity in pediatric population. Caring for the injured child requires special knowledge, precise management, and scrupulous attention to details. All clinicians who are responsible for the care of a pediatric trauma patient, including pediatricians, pediatric emergency room clinicians, and trauma surgeons, must be familiar with every tenet of ...

Hemodilution is not critical in the pathogenesis of the acute coagulopathy of trauma.

BACKGROUND The acute coagulopathy of trauma is multifactorial, but generally believed to be aggravated by coexisting acidosis, hypothermia, and hemodilution. While acidosis and hypothermia have been extensively evaluated, there is a paucity of data on the independent role of hemodilution in this scenario. We therefore hypothesized that hemodilution will impair coagulation following experimental...

Noble-Collip Drum Trauma Induces Disseminated Intravascular Coagulation But Not Acute Coagulopathy of Trauma-Shock.

BACKGROUND There are two opposing possibilities for the main pathogenesis of trauma-induced coagulopathy: an acute coagulopathy of trauma shock and disseminated intravascular coagulation with the fibrinolytic phenotype. OBJECTIVE The objective of this study was to clarify the main pathogenesis of trauma-induced coagulopathy using a rat model of Noble-Collip drum trauma. METHODS Eighteen rat...

The coagulopathy of trauma: a review of mechanisms.

BACKGROUND Bleeding is the most frequent cause of preventable death after severe injury. Coagulopathy associated with severe injury complicates the control of bleeding and is associated with increased morbidity and mortality in trauma patients. The causes and mechanisms are multiple and yet to be clearly defined. METHODS Articles addressing the causes and consequences of trauma-associated coa...
